In GP clinics, polyclinics, and community centers across the region, clinicians can identify concerns, yet a vast care-gap persists that settings cannot close alone. The author contends that closing this gap requires system-wide changes, including care pathways, funding, and support for families and caregivers, to translate detection into sustained treatment and quality of life improvements for patients.
AI and digital health solutions have potential to bridge this divide. Integrated AI-enabled platforms can standardize data across primary and social care, triage referrals, and monitor progression, helping allocate scarce resources and tailor interventions. Predictive analytics and decision support can guide treatment plans, while remote monitoring supports at-home care. Realizing this requires interoperable data, governance, and collaboration among clinicians, policymakers, insurers, and caregivers to ensure privacy and equity.
