Two visions for ambient AI and the future of the EHR

Ambient artificial intelligence entered healthcare as a way to listen to clinical encounters and draft notes. Industry conversations now split into two visions: an evolutionary role in which ambient AI augments documentation and workflow inside existing EHR screens, and a more disruptive model that recasts the EHR as back-end infrastructure beneath an AI-first interface that surfaces insights, automates tasks and orchestrates care.

Both approaches promise efficiency, reduced clinician burden and richer structured data for analytics, but raise challenges around accuracy, liability, bias, privacy, interoperability and vendor competition. Success will hinge on robust validation, standards for data exchange, transparent models and regulatory clarity as early pilots and vendor roadmaps test adoption at scale.
